Bienvivir Senior Health Services is a Member of the National Program for All-Inclusive Care for the Elderly (PACE). Developed more than 30 years ago, the PACE model of care provides an innovative alternative to nursing home care. The common sense approach that the PACE model uses to provide medical care and coordinate support services helps keep people in their homes, close to friends and family. The consolidated funding and interdisciplinary team planning approach makes PACE unique from every other long-term care option and successful in helping seniors remain healthy and independent. To help families understand and learn more about the PACE approach, The National PACE Association has provided a web site, www.PACE4You.org. This easy-to-understand portal provides background on the PACE model of care, allows visitors to view and read testimonials and helps people find a PACE program in their community.
Note: Participants in the program must receive all health care services (other than emergency services) from Bienvivir and may be responsible for the cost of unauthorized or out-of-program services. H451820100517A Date: May 18, 2010
