Bienvivir provides comprehensive health care that is tailored to the needs of
the frail elderly to allow them to continue living in their own home in the
community. All health and health-related services are provided, including
inpatient and outpatient medical care, specialty services and supportive
services with emphasis on preventive care.
Emergency Medical Care is available on a twenty-four hour basis. Bienvivir is an
alternative to institutionalization for the frail elderly who need medical
services and support in an independent setting.
We provide and coordinate the following services based on individual need:

Every enrollee will be assigned to an "interdisciplinary team"
and will become a "participant" in the PACE Program. The Interdisciplinary Team will conduct a comprehensive
assessment of participant's needs and will develop a participant specific Plan of Care that will delineate
the services to be provided to the participant. The participant will be reassessed every six months or sooner
and a new Plan of Care will be developed. The "interdisciplinary team" has the following services/resources
available for application to the participant, contingent on the Plan of Care.
